IRS Tax Relief Financial QA & Support Documents Needed

The IRS offers many tax relief programs based on your current financial situation. We have a Financial QA that serves as a base to measure your current financial status to evaluate a financial-based resolution with the IRS such as an IRS Offer in Compromise, Financial Hardship Status, Hardship Payment Plan, or a Fresh Start Installment Agreement.

You have 2 options in filling out the Financial QA

The first option is to fill it out online & submit it online without having to print or email it. We will automatically receive the Financial QA results when you submit them.

The second option you can download a PDF of the organizer or print it out and fill it out at your leisure. When completed you can email or upload the completed copy to the client portal.

The Financial QA is a simplified financial questionnaire that measures your current finances that will be the basis for a resolution for the back taxes.

What are the Common Supporting Documents needed for a tax relief case?

The common supporting documents that are typically used to submit for tax resolutions & substantiating income & expenses

Some Documents may not apply to your situation. Only submit the documents that apply to you.

  • Copy or Drivers License or State ID

  • Last Filed Taxes

  • IRS Notices

  • 3 months Bank Statements ( 6 months if Self Employed)

  • 3 months paystubs ( P&L Statement if Self Employed)

  • 3 months utilites

  • 3 months car payments

  • Copy of court orders ( Divorce, Child Support..etc)

  • Proof of day care paid

  • Life Insurance Statements

  • Health Insurance Statements

  • Student Loan Repayments

Information on your personal & household infomration, such as name, date of birth, martial status, family information and contact information


SECTION 02

Calcuation of your Monthly Household income.

Whether you are a wage earner, self employed or earn other types of income here is where we calcualte the income on a monthly basis.


Section 03 Monthly Living Expenses

These expenses are what you spend on a monthly basis on personal expenses. Expenses such as Housing & Utilities, Car expenses, Daycare, medical costs, and other expenses.


Section 04 Personal Assets

Your assets that you currently have, real estate, retirments accounts, Stocks bonds, mutual funds & bank accounts.


Section 05 Reasonable Cause & you personal hardships

This section of the client organizer is to detail any hardships, past & present that you have experienced for yourself or an immediate family member.

The government may consider a certain hardship for abatement of penalties or in certain circumstances an abatement of tax.

Sign, Date & Submit the Client Organizer
